New NHS Patients

We are delighted that you have decided to Join Parkway Dentalcare, now what do you do next? Follow the steps below to join our practice.
You are viewing information on how to join the Practice as an NHS patient. We are dependent on NHS England for the funding to see our NHS patients; we endeavour to see all of our patients under their preferred type of dental care but we cannot guarantee this.

Step One – Request our ‘Joining Parkway Dentalcare’ Pack

If you would like us to send you our ‘Joining Parkway Dentalcare’ pack then either:

Telephone – 02392 475086 and speak one of our receptionists or if you reach our voicemail please leave your name and telephone number and we will get back to you.

Mail – We are happy to send out new patient forms in the post to you; Give us a call or fill in our contact form here, to request your new patient applications forms.

Visit – Feel free to drop in and pick up your new patient application forms from our receptionist who will be happy to help you

Step 2 – Complete Our New Patient and Medical History Forms

To enable us to give you the best and safest treatment we need to be aware of all aspects of your health, therefore we require all new patients to fill in a Medical History Form, which your dentist will discuss fully before commencing any treatment.

IMPORTANT – We need these forms before we can book you an appointment.

Then click either download to download the form to your computer or click print to print the form on your printer (if it’s connected to your computer)

Patient Forms

Complete the forms and return to us by post or, if you prefer, scan them, save them as a PDF and attach them to an e-mail with ‘Patient Forms’ as the  subject and your name and contact details in the e-mail.

Alternatively you can complete your medical history form when you arrive for your initial appointment where our receptionist will provide you with any help you need to complete it accurately.

IMPORTANT – Please turn up 15 minutes prior to your appointment if you wish to complete your medical history form before your initial appointment.

Step 3 – Read Our New Patient Pack

Parkway Dentalcare has undergone some changes recently. We now provide different aspect of care, to give our patients the best oral health care available. Please read your new patient pack and asses what provision of care you need.

Please fill out your smile assessment form, which has been included to help us understand your oral health needs so we can provide you with the best care possible.

Step 4 – Making an Appointment

Only once we have received your completed New Patient forms, we will be able to book you an appointment.

For NHS care you will be put on a waiting list, we will then endeavour to contact you and arrange a new patient appointment. We normally contact by letter, email or text depending on your preferred method of communication.

Our Appointment Policy

To ensure the smooth running of our appointment system patients are encouraged to arrive promptly for their appointments. NHS guidelines require that NHS patients give 24 hours’ notice if you wish to cancel your appointment. If you cancel or fail to attend 2 or more appointments we will no longer be able to offer you NHS treatment. We will of course take any special circumstances into account.

Our practice policy for private treatment requires 48 hours’ notice to cancel appointments. If you are unable to attend without prior notice you may incur a late cancellation or failed appointment fee.
